Visiting Our Clinic

Please add at least 30 minutes to an initial session to find parking and locate our consulting suite.

You will need to complete a questionnaire prior to your appointment. 

It can be downloaded HERE and emailed prior to the appointment with referral to:

To avoid missing your appointment, please note this clinic runs to time and please telephone if you need directions or have a late ETA. 

If you have a complex case, it might also be worth considering writing down any specific questions to avoid forgetting, and bringing a support person.  

You will require an up to date GP referral, Medicare and private health insurance.
